Hopefully your primary concern is getting your problem fixed and staying active and well, not the finances of how much your care will cost. I agree finances are important and I personally put in writing all necessary treatment costs, minus your insurance benefits, and list your financial responsibility as the patient.
But I have not yet answered the question of cost. That's because everyone is uniquely different and every condition has various complications which range from mild to severe. Lastly, every insurance company has a whole rainbow of different policies* (some with deductibles, co-pays, co-insurances, and hidden limitations).
THEREFORE, UNTIL WE CHECK OUT YOUR CONDITION AND CALL YOUR INSURANCE IT IS IMPOSSIBLE TO GUESS WHAT YOU WILL OWE EACH VISIT.
